Caravan seem to choke going up hills. The whole van jerks. 170,000n miles. help

Van seems to run fine on city streets but interstate travel and going 60 to 70 mph seems to make van choke and lunge a bit. Usual mechanics did a test drive but can't fix it. Fuel pump was changed last year. any ideas??? Has been a great van until the choking problem.
