also check the brake switch for function. It is usually on, or near the brake pedal. From what I can tell, a simple continuity test will tell you if it is working. If you have replaced any bulbs recently, make sure they are the correct bulbs

I had the same problem on my 97 voyager. I checked the fuses and bulbs and both were fine. I checked the actuator (the button that is depressed when you step on the brake) and that seemed to be fine too. The ACTUAL culprit was the electrical contacts on the actuator. There is a group of wires that connect by a strip to the mechanism that is attched to the brake pedal. The contacts on that group were just slightly rusted, so i cleaned with a metal nbrush and fit them back in. Worked like a charm!

Have you tried replacing the brake light bulbs? Not meaning to be sarcastic, just asking the first and most obvious question for your problem. If you have done that already, you may have a problem with the Electronic Control Module, since the mid 90's Chrysler mini-vans are rather famous for wonky electrical problems.
