I have seen the pilot bearing cause this noise sometimes only when you first start the engine in the morning, perhaps a bad clutch pressure plate release bearing.

Hearing gravely noise when driving in all gears except fourth and only stops completely when clutch in. Had transmission bearings replaced last year, same noise as before just not as bad!!???
when not in gear at rear of the transmission has a low grinding or growling sound and after shifting into gears 1, 2, 3, and 5th gear continues to make grinding noise but 4th gear sounds perfect....
with the truck running I can shift thru all the gears and also from 2 to 4 wd it does the same thing I don't have to use the clutch and can go thru all gears with no kind of grinding or hesitation,...
it started three days ago,it feels like it's stuck in 2nd gear like when going up hill but it's like that all the time.
