is the timing chain tensioner located on the outside of the engine or does the timing chain cover (etc) have to be removed first?

MasterTiming chain tensioner is rear most mid of the timing cover secured by two nuts/studs. It extends and puts pressure on the timing chain tensioner slipper. I have seen timing chain/sprocket wear but no bulletins from Toyota about it.
