A hissing sound could quite possibly be a vacuum leak from the manifold or throttle body gasket, it should be easy to find the source of the leak, repairing it how ever is more expensive it it is the intake manifold gasket. the reason the idle surges is the computer in your car is trying to compensate and constantly correct the idle speed but the vacuum leak is not allowing the computer to work within its parameters.
