My clutch pedal goes all the way to the floor and when I barely lift it up the gear engages. I am also having trouble shifting in any gear at times. I have done a little bit of research and think it is my slave cylinder. Are my assumptions correct or is it something else?

It is a common failure, but to say that is your issue, we can't say that it is without checking it. It could also be the clutch m/cyl, and even possibly the clutch itself. Get it verified.

For my truck is the clutch m/cyl and the slave cyl all one unit or are they two separate parts?

2 separate parts that tie together. I don't think they offer the complete assy, you might check.
