On higher end cars I have always got value out of extended warentees if they are any good. Have someone (apart from the salesman selling the extended warentee) read the fine print of what is or isn't covered. A friend of mine had an extended warentee that would cover a cruise control computer (trivial) but not cover a failed fuel injection computer (important), another warentee I looked at would cover automatic transmission gaskets and seals but no internal transmission parts.
