With an angle head drill and a good extractor set. Bad scenarios involve removing the cylinder head and having a machine shop extract them.

i work in a parts store that has a machine shop. those exhaust manifolds warp all the time and snap bolts. they are okay once they are machined flat again.

On the 3.7L V6 and 4.7L V8 the Exhaust manifold bolts commonly break, exhaust manifold gaskets should be replaced and replace bolts as needed.
Gas mileage is bad, black tailpipe. O2 sensor light still comes off/on after replacing O2 sensor. Ticking noise passenger side of engine. 42000 miles.
