Driver side wiper arm comes off. on Chevrolet Impala

Wiper arm came loose and would not turn during heavy snow storm. Fixed by pulling cap off nut holding arm down. Ensure arm in correct place and tighten down nut, then replace cap.
Wiper arms comes off during rains. Repeated tightening has not solved the problem.
Mine came off during rainstorm just after having new wiper blades installed. Now the nut will not stay tight.
WIPER ARM That control the blades has came off the ball.

After a bad snow storm tried to turn on my windshield wippers and the wouldnt tunr on. After a few days to allow time for the snow to melt and etc, only the passenger side would completely come on, then it proceeded to over lap with the drivers side and the blade completely came off. What cluld possibly be wrong, and what can be done to fix it

Went to tighten nut to see if it worked, but nut wasn't loose. Both wiper arms not working. Passenger comes up a couple inches and falls. Driver side arm goes off the windshield to side mirror
