when the van is started, it sounds very loud in the engine like a clacking sound. It goes away after about 5 minutes. the dealer tells me its a tensioner issue and should be resolved with regular oil changes using high grade oil. any help?

How many miles are on your Quest. Has the oil been frequently changed? Ensure your engine oil is changed every 3000 miles and that in between service the oil level is kept at "full".
Nissan have a service bulletin issued July 2007 TSB NTB07-042 regarding a buzzing whirring sound form the timing chains that details replacing the timing chain to rectify the problem.
