My van smells like fumes inside, does it need to have the cabin air filter replaced? How? If not what else could it be?

    i would look for the cause. could be a fuel leak, oil leak or coolant leak. have it checked by a shop to verify.
    the cabin filter is under or behind the glove box. easy acess after removing the glove box.

    What does it smell like? If it's sweet then it's probably coolant. Fuel smells are distinctive but oil leaks may smell burned or like someone tarring a roof. You need to inspect everything in the engine compartment looking for oil, coolant and fuel leaks. If you're unsuccessful then bring it to a shop nearby. The cabin filter is a simple replacement that should be done every 30K miles with the air filter. Good luck.
